Well, the stuff you download online is created by amateurs, so if they can do it so can you. I think you need to know how to create a mesh and I heard some people just use Photoshop. You could always go on Sims-related message boards and ask there.

Like any other clothing for The Sims 2 you will need something to create the form of the clothing (a mesh) and something to create the color and appearance of the clothing (a texture).
You can make both of these yourself, but it does require some work to make it look nice.
I suspect you will also have to use SimPE to make sure that the item you make will appear when your sim is pregnant.
Here’s a starter set of tutorials about clothing in general. I’d start here to get myself familiar with the concepts and how to do basic clothing mods.
